Your Visit
Dr. John Stock has been providing the best medical care to children with heart diseases since 1994.
The dedicated staff of Pediatric Cardiac Care of Arizona will schedule appointments and procedures as efficiently and timely as possible.
Your visit will include a detailed, history, physical examination, a full discussion of findings, and an understandable care plan.
For new patients, please fill out the registration packet as well as the new patient form that corresponds to the patients age.
We request that you plan to arrive 30 minutes prior to your scheduled appointment. This gives us time to obtain vital signs and complete the EKG prior to starting the appointment.
Please check with your insurance to make we are “in network” with your particular plan.
Quality care is dependent on good communication. We encourage you to bring in information concerning your child’s cardiology care and cardiology tests performed by other physicians. Patient Forms
Dr. Stock will communicate with your provider a summary of the findings. Together with you, we will determine the best plan for further evaluation, testing, and follow up (if necessary).
